Health Insurance in Korea for Foreigners: Enrollment, Premiums & Disputes
A foreigner living or working in Korea needs to understand how the National Health Insurance (NHI) system works — how to enroll, use the insurance card, and what to do if there is a dispute over premiums or benefits.

What Korean law says
- ·Art. 12 §1 (National Health Insurance Act): NHIS must issue a health insurance card when a subscriber or dependent requests one.
- ·Art. 12 §3: A subscriber or dependent may present a passport, driver's license, or other ID recognized by the Ministry of Health and Welfare instead of the health insurance card, as long as the medical institution can confirm eligibility.
- ·Art. 89 §1: The Health Insurance Dispute Mediation Committee (분쟁조정위원회) is established under the Ministry of Health and Welfare to review and decide on appeals (심판청구).
- ·Art. 89 §3–4: The committee convenes with 9 members per session; decisions require a majority of attendees, with non-public-servant members forming the majority.
- ·Art. 4 (National Health Insurance Act): Premium rates for employed subscribers (직장가입자) and regional subscribers (지역가입자) are deliberated by the Health Insurance Policy Deliberation Committee under the Minister of Health and Welfare.
Required conditions
- ·You must be a registered NHI subscriber (직장가입자 or 지역가입자) or an eligible dependent (피부양자).
- ·To use ID instead of insurance card at a medical facility: the facility must be able to confirm your eligibility from the ID presented (Art. 12 §3).
- ·To file a dispute appeal: a prior formal objection (이의신청, Art. 88 — referenced in Art. 89) must typically be completed first; check with NHIS for exact procedure.
- ·For the SME social insurance premium tax credit (Art. 30-4, Special Tax Treatment Control Act): only applicable to small and medium-sized enterprises with an increase in full-time employees compared to the prior year, up to taxable year ending Dec 31, 2024.
What to do next
- 1Step 1 — Enroll: If employed, your employer registers you as a workplace subscriber (직장가입자). If self-employed or not employed, register as a regional subscriber (지역가입자) at your local NHIS branch.
- 2Step 2 — Get your insurance card: Apply at any NHIS branch or online (nhis.or.kr) — free of charge (Art. 12 §1).
- 3Step 3 — Using healthcare: Bring a valid ID (passport, alien registration card, etc.) to any 요양기관. No insurance card needed as long as the clinic can confirm your eligibility (Art. 12 §3).
- 4Step 4 — If you disagree with a premium or benefits decision: First, file a formal objection (이의신청) with NHIS. If still unsatisfied, file an appeal (심판청구) with the Health Insurance Dispute Mediation Committee (Art. 89 §1).

Common mistakes
- ·Assuming you must carry the physical insurance card — under Art. 12 §3, a valid ID (passport, alien registration card) is legally sufficient.
- ·Not enrolling as a regional subscriber (지역가입자) when between jobs or self-employed, leading to unpaid premium arrears.
- ·Missing the objection (이의신청) step before filing a formal appeal (심판청구) — these are two separate stages.
- ·Confusing the 5-year comprehensive NHI plan (Art. 3-2) with individual entitlements — the plan sets policy direction, not personal rights.

제102의2조 (건강보험공단에 출연) · 국민연금법
제102조의2(건강보험공단에 출연) ① 보건복지부장관은 연금보험료 등의 징수에 소요되는 비용을 제103조에 따른 국민연금기금운용위원회의 의결을 거쳐 기금에서 건강보험공단에 출연할 수 있다. 이 경우 출연금의 규모, 기준 등에 관하여 필요한 사항은 대통령령으로 정한다. ② 건강보험공단은 제1항에 따른 출연금에 대하여 결산상 잉여금이 있을 경우 제45조를 준용한다.
제3의2조 (국민건강보험종합계획의 수립 등) · 국민건강보험법
제3조의2(국민건강보험종합계획의 수립 등) ① 보건복지부장관은 이 법에 따른 건강보험(이하 "건강보험"이라 한다)의 건전한 운영을 위하여 제4조에 따른 건강보험정책심의위원회(이하 이 조에서 "건강보험정책심의위원회"라 한다)의 심의를 거쳐 5년마다 국민건강보험종합계획(이하 "종합계획"이라 한다)을 수립하여야 한다. 수립된 종합계획을 변경할 때도 또한 같다. ② 종합계획에는 다음 각 호의 사항이 포함되어야 한다. 1. 건강보험정책의 기본목표 및 추진방향 2. 건강보험 보장성 강화의 추진계획 및 추진방법 3. 건강보험의 중장기 재정 전망 및 운영 4. 보험료 부과체계에 관한 사항 5. 요양급여비용에 관한 사항 6. 건강증진 사업에 관한 사항 7. 취약계층 지원에 관한 사항 8. 건강보험에 관한 통계 및 정보의 관리에 관한 사항 9. 그 밖에 건강보험의 개선을 위하여 필요한 사항으로 대통령령으로 정하는 사항 ③ 보건복지부장관은 종합계획에 따라 매년 연도별 시행계획(이하 "시행계획"이라 한다)을 건강보험정책심의위원회의 심의를 거쳐 수립ㆍ시행하여야 한다. ④ 보건복지부장관은 매년 시행계획에 따른 추진실적을 평가하여야 한다. ⑤ 보건복지부장관은 다음 각 호의 사유가 발생한 경우 관련 사항에 대한 보고서를 작성하여 지체 없이 국회 소관 상임위원회에 보고하여야 한다. 1. 제1항에 따른 종합계획의 수립 및 변경 2. 제3항에 따른 시행계획의 수립 3
